TMS - Add gross revenue to driver settlements

User Issue:

Sometimes users would like to be able to add the gross revenue from the load to the driver settlement so that the driver can see it along with their driver pay.

Solution:

Users can now check a box to add the gross revenue to the driver settlement when they are creating a new one.  This is for TruckingOffice Pro users only.
