Update Rate Confirmation

I think that the rate confirmation is due for an update. One of the things I notice is that the name of the broker is on the bottom of the rate confirmation and looks very random. Also, the carrier information should be located at the bottom near the rate and carrier signature. 

I also make sure to add custom text with detailed instructions and one of the things that looks a little messy is that it's at the bottom of the signature area. I think it would look a lot cleaner if it was on its own separate 2nd page vs some text on the first page and a continuation of the text on the 2nd page.
